Location: Outskirts of Chicago, IL
Reports to: Director of Health Services
Compensation range: $85k - $95k Base compensation
Position Summary:
Provide direct nursing care to residents and delegate and supervise the care and services provided by Licensed Nurses and Quality of Life Specialists. Administer medications, perform treatments and procedures, obtain specimens as needed, and complete documentation as required. Assist the DOHS in ensuring community adherence to all applicable federal, state, and local standards and regulations promoting regulatory compliance.
Responsibilities:
Provide daily supervision to assigned staff and directly provide nursing care to residents as assigned.
Supervise nursing personnel on all shifts and document resident status as per policy.
Participate in staffing, scheduling, counseling, orientation, and evaluation of nursing staff.
Participate in the admission of new residents and assist residents with Activities of Daily Living (ADL), family/social activities and actively promote.
Responsible for identification of resident medical and behavioral changes and the development and monitoring of care plans and re-assessment of each resident.
Provide daily report to the Director of Health Services (DOHS) concerning facility conditions, personnel performance, resident conditions, and needs for nursing supplies.
Participate in daily meetings in the absence of the DOHS.
Provide nursing care according to physician’s orders and in conformance with recognized nursing practice, established standards, and administrative policies.
Assist the DOHS with the auditing of resident records to ensure adherence to community policy and procedures and regulatory compliance.
Ensure preventative, therapeutic and rehabilitative services to residents and assist in the coordination of nursing care with care rendered by other disciplines.
Make rounds with physicians and assist in examinations and/or treatment of residents.
Assess residents and notify attending physician, and responsible party in the event of a change in the resident’s condition.
Accurately administer prescribed medications, supervise the continuity of nursing care, respond to changes in medical condition, medications or other circumstances requiring special care needs and perform first aid as required.
Provide daily supervision and leadership to assigned staff and other designated personnel, ensuring resident needs are met.
Provide written evaluation and oral counseling of assigned staff.
Assess and monitor complex resident health status and determine the appropriate nursing intervention.
Assure that all resident records are accurate and complete.
Assure that nursing care is practiced in full accordance with regulatory requirements, facility quality standards, and resident rights.
Observe and abide by all regulations to ensure that personal health information is protected during its collection, use, disclosure, storage, and destruction within the community.
Consistently work cooperatively with residents, co-workers, physicians, families, consultant personnel and other ancillary service providers.
Assist in orientation of nursing personnel.
Qualifications:
Current Practical Nursing License (LPN) in the state of Illinois, in good standing, is required.
Minimum LPN degree in Nursing required.
Minimum on (1) year experience in Nursing experience in Assisted Living, Memory Care is required.
The ability to speak, read and comprehend the english language.
Knowledge in all areas of local Health Department, OSHA, and other regulatory agencies relevant to healthcare services.
Effective communication skills (oral and written).
Good inter-department communication and teamwork skills.
